The utilization phase of Green Building (BGH) plays an important role in ensuring the sustainability of building performance through effective control of energy consumption, water efficiency, and operational management. RSUD Tanjung Redeb, Berau Regency, as a healthcare facility operating 24 hours a day, has a high level of energy and resource consumption, requiring an operational management strategy based on green building principles. This study aims to analyze the operational performance of Green Building implementation during the utilization phase at RSUD Tanjung Redeb and evaluate the potential for energy efficiency, water conservation, and green building certification achievement. The research method used is descriptive qualitative with a technical analysis approach through observation of mechanical, electrical, and plumbing (MEP) systems, interviews with facility management personnel, and review of supporting documents such as utility logbooks, maintenance standard operating procedures, and green building assessment regulations based on the Minister of Public Works and Housing Regulation No. 21 of 2021 and the Ministerial Circular Letter No. 01/SE/M/2022. The analysis was conducted using Energy Consumption Intensity (IKE), water efficiency performance, and Green Building assessment score accumulation parameters. The results indicate that the implementation of high-efficiency chillers, LED lighting systems equipped with occupancy sensors, greywater recycling systems, and rainwater harvesting technology can improve operational efficiency at RSUD Tanjung Redeb. The building is projected to achieve an IKE value of 280.78 kWh/m²/year, representing an energy efficiency improvement of 26.11% compared with the standard limit for hospital buildings. Furthermore, water conservation strategies are projected to reduce water consumption by 26.92% compared with the baseline condition. From the financial perspective, the implementation of Green Building strategies provides potential operational cost savings of approximately IDR 2.55 billion per year with an estimated investment payback period of 5–7 years. Based on the performance assessment model, the operational achievement score is projected at 73.04%, indicating the potential to meet the criteria for a “Madya” level Green Building certification. This study demonstrates that optimizing the utilization phase is a key factor in maintaining green building performance while supporting cost efficiency, resource conservation, and sustainable healthcare facility development.
